Hey quicksilver,
My buddies phone is having the same problem:
The phone won't recognize the sd card ever. I tried several cards and formatted them on my computer. This problem first happened when i was in cw recovery and had the sd card mounted to the computer through usb storage. for some reason it didn't eject properly and my computer said error. from there i was unable to get the sd card to mount yesterday. i have searched every forum so far and it doesn't look to good. It boots into recovery when it is plugged in the computer but other than that I can only access hboot. It now says S-ON even though it was S-OFF going into this. Its running the evo radio so I doubt he can exchange it at verizon as it can be seen from hboot as 11.19. It notices the sd card though while in hboot and loads it but it wont touch the update image PMx31.zip (forgot name).

Flash the stock RUU zip for the incredible. and then you will be able to boot it when its plugged into the computer atleast. from there i think you might be able to fix it quick silver. Do this from HBoot
Atleast then if you can't fix it you have a stock s-on phone to exchange

Yea great idea, thanks. It worked, though it still has the same problems. Now that it's stock I think I'm going to have to exchange since I can't find any solutions anywhere.

Not sure how useful this is at this point, but I suppose additional data points are never a bad thing. My (brand new) Inc did this right out of the box. Same symptoms: would boot fine when plugged into USB, but couldn't be recognised by the computer, and booting when unplugged resulted only in the five vibrations/flashing light. Interestingly, the phone couldn't recognise its internal storage even after a successful boot, nor did Bluetooth work. The Verizon store employee took one look at it and brought me a new phone.

I'm having this 5 vibration-and-then-green-light problem too, with it not recognizing an SD card in the slot... But here's the worst part: Bootloader won't recognize the SD card either, so I can't flash the stock RUU (Or anything for that matter) onto the phone =\
Also I had it in S-Off but on the rare occasion that I DO get the Bootloader to start, it says S-on... The phone will usually boot up into Redemption if it's plugged in when I turn it on, though. Another thing is if it's plugged into a computer, when I unplug it it'll do the 5 vibration thing again and keep flashing the green LED till I pull the battery out... Any suggestions on that?
P.S. I've also tried HTC's RUU program (The one that uses HTC Sync) to no avail, and also tried the instructions in post #7 here, still no dice. (Won't let me push any files using adb, just says "error: device not found"
Any help would be greatly appreciated as I'm not sure I can go to Verizon or HTC with it.

To my knowledge, we have not been able to recover anyone from the 5 vibration/green light problem. I recall Doug mentioning doing an immediate battery pull then flashing a new radio as soon as this happens, and that might work. You may be past that stage but worth a shot I suppose if you can get into the bootloader again.
Sorry but I think Verizon is your only option at this point.

Did anyone got any success in fixing the INC, with the 5 vibrate problem.

Those who are having problem using their incredible, after the 5 vibrate thing. Wish me luck if htc doesn't replace my phone. I will most likely comeup with a fix for you guys. Its definately going to be hardware though.

Sorry guys for not getting back I thought I was the only one with the problem.
So what happened i called the HTC or the parent company they told me it was a hardware problem nothing Verizon could do I was out warranty, HTC said if I mailed my phone to them they'll "fix" it but I wouldn't have a phone for two weeks or longer.
So basically only way to fix this problem is Verizon will replace it under warranty or shipping it to HTC.
I just bought a cheap fascinate til something new comes along.

Alright... Went to Verizon with the phone with it and got a new one the next day 
Unfortunately this "like-new replacement" has a problem with the display turning off (touchscreen works but screen turns off sometimes) lol, going back to get yet another one...
Third time's the charm right?
Still I'd like to know what actually causes the problem, anyone have any info on that? I mean its not like it happened the first time I'd flashed a ROM, I did it with no problems about 6 times before, and it was the second time I flashed that particular one.

this happens to me to exactly. i took it in to sprint and they said that because my phone has "water damage" it cant be covered under my warenty.
