Calls ringing again after dismiss / dismiss not sending caller to voicemail?

I'm not sure if anyone else has had this issue, but I had trouble finding info on it searching here and google. Here's what's happening:

Someone calls me, I dismiss it, and the call ends (for my phone, anyway), and then they immediately call me back. So I had a theory that they never got sent to voicemail on the first set of rings. I just confirmed it by calling myself from another phone, and indeed the caller just continues to hear many rings, and after I screened the 'second' call, they finally get pushed over to voicemail.

It's driving me crazy, though, because I can't always answer, but when someone calls twice I assume it's important - and twice now I've left meetings to answer it only to find out that they didn't call twice and it wasn't urgent.

I'm currently on MIUI 2.3.2 (android 2.3.5) on a rooted Droid Incredible (ADR 6300). Baseband 2.15.00.07.28, Kernel 2.6.37.6-CyanogenMod+. I've also experienced this same issue on CM7 and MIUI 1.12.9.

Do you know if this might be MIUI specific? I used MIUI constantly but seem to remember some little glitched like that on my Incredible the last time I had tried it. It might be worth doing a ROM swap, trying the experiment again, and see what happens.

Unfortunately, I have confirmed that it happened on both versions of MIUI (2.3.2 and 1.12.9) as well as Cyanogen 7.1.0. I am not certain if it happens consistently since I am not usually calling myself, but every time I test it, it happens. Also, it seems that silencing the ringer and dismissing the call cause the repeat rings. I will confirm today if letting it continue to ring without silencing or dismissing causes repeat rings, but it does not seem to (which is very strange).

Update - it does not appear to be consistently happening (at least not any more) as I just tried again, and did not get double rings. I'm still very curious to see if anyone else had this issue though.

That is strange. So on MIUI and Cyanogen. MIUI is Cyanogen based still, isn't it? Or did they break away? Can you try a regular Sense ROM like Skyraider or the sort quick to see if it still happens there?

If I get some free time tonight or tomorrow I will definitely try restoring back to an old SkyRaider and testing.
